Insurance & Billing

Fitness Physio accepts a broad range of insurance plans and handles verification and coordination for you. If your provider isn’t listed, our team will confirm your benefits and guide you through your options.

Certain plans require prior authorization for physical therapy. We manage that process on your behalf. If care continues during review or after a denial, sessions are $95 per visit and reimbursable if later approved. We recommend staying in contact with your provider to ensure approvals are completed.

Before your initial visit, our team will review your insurance plan to identify physical therapy benefits. You will be informed about your coverage details, including any co-pays required at each visit. You can pay co-pays via check, credit card, cash, and HSA or FSA cards.

Verification of insurance benefits is not a guarantee of payment. Any balance not covered by insurance is required to be paid by the client. We attempt to verify as a courtesy. If you are unsure about your coverage, please call your provider prior to any treatment.

BILLING

Special Billing Situations

Medicare

Physical therapy is covered up to Medicare-established thresholds. Any care beyond that point will be discussed in advance, with payment collected at the time of service until additional coverage is confirmed.

Worker’s Compensation

Care is covered in accordance with state guidelines. Our team coordinates directly to ensure your claim is handled efficiently and without added friction.

Self-Pay

For those who prefer direct access or a more elevated, one-on-one experience, we offer self-pay options designed around performance, recovery, and results.

  • 45-Minute Treatment Session – $95
  • 20-Minute Performance & Recovery Session – $50

If you choose to self-pay while insured, these visits will not apply toward your deductible and cannot be submitted to insurance after the fact.

Seamless Auto Billing

We offer secure autopay for a streamlined, premium experience. Your card is safely stored and charged after each visit, and once claims are processed, no follow-up is required. All bills will be sent via email.

Receipts are delivered automatically, with detailed statements available upon request.

Please allow 6-8 weeks for insurance claims to process.

Cancellation

We operate on a high-performance schedule to maximize access and results for every client.

A minimum of 24-hour notice is required for any cancellation or change. Missed or late-canceled appointments are subject to a $50 fee charged to the card on file.
